Your six-step route

What the whole thing looks like

Do not start the form until the preparation and payment steps are complete.

  1. Create clear files for every required document.
  2. Transfer exactly RM9.72 and save the receipt.
  3. Use the official referral link on this page.
  4. Enter Parts 1–4 using the exact settings shown.
  5. Match each document to the correct upload slot.
  6. Ignore the duplicate-payment prompt and notify Lucas.

Prepare every document first

Use clear PDF or image files. Names, IC numbers, addresses and bank account details must agree across all documents.

Every file must be readable at normal zoom. Upload the complete page — do not crop off your name, bank logo, statement month, account number or address. Do not upload transaction-history screenshots instead of formal bank statements.

Make the RM9.72 CDS payment once

Make this payment before starting, so the receipt is ready for the upload section.

If you register a Maybank account and submit Maybank statements, the RM9.72 payment must also come from that Maybank account. Do not pay from another person’s account or a different bank account.
After you submit the application, the last page may display another RM9.72 payment request. Do not make a second payment. Section IX shows what to do when that message appears.

Complete and sign the W-8BEN form

This form is used to declare that you are not a U.S. citizen or U.S. person for tax purposes.

Write your name and address, then sign the form. Do not write the date. Do not alter other sections unless Lucas or UOB Kay Hian gives you an updated instruction.

Final W-8BEN check. Use the same spelling and address as your IC and application. Make sure the signature is visible and the scanned page is not cut off.

If someone else supports your income

Use this route for a student, retiree, homemaker or any applicant who relies on an income provider.

Prepare these items

Use consistent information

The name and IC number on the consent form must match the provider’s IC. Enter the same employment details in the online form.

If you are unsure whether you need an income provider, stop before submitting and confirm with Lucas.

Only the branch, dealer and reference values in this guide are fixed. All names, IC numbers, phone numbers, addresses, employment details and bank details must be your own.

Choose the Cash Upfront Account settings

Select the following values unless you intentionally want a Shariah-compliant account.

Before clicking Next: check that Cash Upfront is selected. A different account type may have different limits, financing terms or documents. When all four selections are correct, click Proceed to Next Section.

Parts 2 to 4 — personal, financial, acknowledgement

Part 2 — copy your IC address exactly

Your personal details must match your NRIC / MyKad. Do not use nicknames or shortened addresses. Copy the address exactly as printed on your IC. Keep the same line order. Do not add commas or unnecessary spaces.

If the IC has three address lines but the portal has only two, combine the IC’s second and third lines in the portal’s Address Line 2.

If your current address is not the same as the IC address, answer the relevant question truthfully and enter the current address in the separate residential-address section.

Part 3 — match the bank name, account and statements

The bank account entered here must be the account shown on your statements and used for the RM9.72 payment. If the statement says Maybank Islamic, select Maybank Islamic — not Maybank. Copy the full account number exactly. Do not add spaces unless the portal formats them automatically.

Part 4 — read every declaration before selecting the answers

Do not tick boxes blindly. Confirm that the statements are true for you.

For the question asking whether you want to appoint and empower your dealer’s representative to manage administrative functions for your trading account, select YES and tick MY DEALER Representative.

For all other acknowledgement questions, answer according to your actual circumstances. If a statement is unclear, pause and ask Lucas before submitting.

Part 5 — put the correct file in each upload slot

Open each uploaded file once after attaching it. Check that it is the right document and remains readable.

No income proof?

Use the income-provider route in section V and upload the documents requested for that route.

Before leaving this page

Every required slot must show the correct file. Do not upload a password-protected PDF unless the portal specifically accepts it.

IX

Review, submit, then stop at the payment prompt

The portal may not enable submission until every required agreement has been opened and accepted.

  1. Tick the declaration or agreement checkbox.
  2. Open the document that appears.
  3. Read it and scroll all the way to the bottom.
  4. Click Agree. Repeat for every required agreement.
  5. Review all information and attached files one final time.
Do not rush the final review. A wrong IC number, bank account, email address or unreadable file can delay the application. Correct mistakes before clicking Submit My Application.
If you already paid RM9.72 and uploaded the receipt, ignore the Make Payment Now message. Do not pay RM9.72 a second time. Save the application confirmation or reference number if one is shown.

If you did not make the first RM9.72 payment, or you are unsure whether it succeeded, do not guess. Contact Lucas before taking any further action.

After submission

Send the completion message only to the confirmed contact number, with your name, IC and the reference QJJ.

What happens next

  1. Keep the application confirmation and payment receipt.
  2. Check your inbox and spam folder for UOB Kay Hian emails.
  3. Respond promptly if additional or clearer documents are requested.
  4. Create your user ID and password only through the official UOB Kay Hian instructions.

Use only the confirmed number. Do not send your password, OTP, login link, signature file or bank password. UOB Kay Hian will send an email when the account is processed.
